Muhurta for a Medical Procedure and Surgery

The muhurta of treatment and surgery is the branch in which the classical texts are most cautious. The "Muhurta Chintamani" gives operations a chapter of their own, and the first thing it stipulates is that urgent intervention is not bound by the rules of time selection. Choosing a date makes sense only for a planned procedure, where the doctor leaves a choice.

That caveat stands first for a reason. Everything else in the branch is a refinement inside the freedom medicine leaves. If there is no freedom, there is no muhurta; if there is a choice of several days, the rules below help to make it.

The rule of the Moon and the part of the body

The chief special rule of this branch: do not operate on the part of the body ruled by the sign the Moon occupies at the time. The signs are matched to the body from the head down — Aries: the head; Taurus: the face and neck; Gemini: the shoulders and arms; Cancer: the chest; Leo: the heart and stomach; Virgo: the abdomen and intestines; Libra: the lower back and kidneys; Scorpio: the pelvis and the organs within it; Sagittarius: the thighs; Capricorn: the knees; Aquarius: the shins; Pisces: the feet. While the Moon passes through the corresponding sign, intervention in that area is postponed.

The rule is convenient in needing almost no calculation: the Moon crosses a sign in about two and a quarter days, so the awkward stretch lasts around two days and returns once a month. Avoiding it for a planned operation is usually easy. Some schools extend the prohibition to the sign opposite the affected area, and to a day on which the Moon is conjunct malefics — but the basic rule is the first one.

Waxing and waning Moon

For removal — a growth, a tooth, an organ — a waning Moon is taken: waning is associated with decrease and cleansing. For restorative and plastic work and for anything that must take and grow, a waxing one. The same distinction is applied to the tithis: the waning cycle for removal, the waxing for restoration.

The same rule serves in small matters: removing moles, cleaning, drawing out on the waning Moon; implantation, augmentation, the taking of a graft on the waxing. Dentistry divides by the same sign: extraction on the waning Moon, prosthetics and implants on the waxing.

What is examined in the chart of the moment

The lagna and the Moon stand for the patient and must be strong: the lagna unafflicted, its lord not combust and not placed in the sixth, eighth or twelfth house. The sixth house answers for the illness and for the treatment, the eighth for risk and for a long recovery, the twelfth for the hospital and for expenses. The seventh house in this muhurta is read as the doctor and the surgeon.

Mars is the natural significator of surgery, of the instrument and of blood; for an operation his strength counts as a good thing, and his affliction by Rahu or Saturn does not. Saturn answers for chronic conditions and for a long convalescence, Jupiter for recovery and a favourable outcome, so his aspect on the lagna or the eighth house is taken as the best confirmation of the choice. A badly afflicted eighth house of the moment is the chief sign for which a date is moved if that is possible.

Auspicious guidelines

Nakshatras: Ashwini — the nakshatra of the Ashwins, the celestial physicians, singled out for treatment — as well as Mrigashira, Punarvasu, Pushya, Hasta, Chitra, Anuradha, Revati. Weekdays: Tuesday is permitted and even preferred for surgery as the day of Mars, the natural significator of cutting and of blood; Wednesday for diagnosis and therapy.

This is the rare branch in which Tuesday is auspicious: in the other muhurtas it is taken reluctantly. The reason lies in the nature of the event — an operation is intervention with a sharp instrument, and that is exactly what Mars answers for. For beginning drug treatment, on the contrary, Wednesday and Thursday are preferred: Mercury answers for composition and dosage, Jupiter for recovery.

Among the tithis 2, 3, 5, 7, 10, 11 and 13 are preferred; the rikta tithis — the 4th, 9th and 14th lunar days — are excluded. Of the yogas, Vyatipata, Vaidhriti, Parigha, Vyaghata, Shula, Ganda, Atiganda and Vajra are not used. Rahu kala is reckoned from local sunrise for the city where the procedure takes place.

Matching the patient's chart

Chandra bala is checked: the transiting Moon must not stand in the fourth, eighth or twelfth house from the patient's natal Moon. Tara bala is checked: the nakshatra of the day must not fall into janma, vipat, pratyari or vadha from the natal nakshatra. Both calculations rest on the natal Moon and need a correct birth time — if it is approximate, it makes sense to go through rectification first.

What is avoided

The karana vishti, amavasya, eclipses, gandanta, rahu kala, ashtami and chaturdashi. A time with an afflicted lagna and eighth house of the moment, and with the Moon conjunct malefics, is not taken. One does not operate on sankranti, the day the Sun enters a new sign.

Days with a combust Moon close to the new moon are not used either: in the muhurta of treatment the Moon answers for vital force and for how the body will bear the intervention. A waning Moon of the last quarter is not taken for restorative procedures, though it suits removal.

Beginning treatment and medication

Besides surgery the classical texts treat the beginning of treatment in general — the first dose of a medicine, the start of a course, the first visit to a doctor. The rules here are milder: it is enough to avoid vishti, amavasya, eclipses and rikta tithis, to take one of the auspicious nakshatras and to see that the Moon is not in the eighth or twelfth from the natal one. Among the days Wednesday is preferred — the day of Mercury, who answers for composition and measure — and Thursday, the day of Jupiter and of recovery.

Ashwini, Pushya, Hasta, Mrigashira and Revati count as the best nakshatras for beginning treatment: all belong to the swift and soft groups, and Ashwini is directly connected with healing. A long course meant to run for months is begun on a waxing Moon; cleansing procedures and everything meant to carry something away, on a waning one.

Prevention and examinations

A routine examination, tests and diagnostics are not usually given a muhurta: this is not the beginning of an undertaking but the obtaining of information. The only thing tradition advises even here is not to schedule them for rahu kala or vishti if the time is yours to choose. Neither postponing an examination nor interpreting its result by the chart of the moment is appropriate.

It should be said separately what must not be done in this branch: determining the presence of an illness from a chart, cancelling prescribed treatment, or altering a dose. All the rules above concern the choice of time within medical advice, and they have no other role.

The order of work

• ask the doctor which days are available at all — that is the outer frame;
• establish the nature of the intervention, removal or restoration, and choose the lunar phase accordingly;
• exclude the days on which the Moon stands in the sign ruling the area to be operated on;
• discard vishti, amavasya, eclipses, sankranti, gandanta and rikta tithis;
• check the patient's chandra bala and tara bala;
• within the day find a stretch with a strong lagna, a clean eighth house and an aspect of Jupiter, outside rahu kala.

The main caveat

Astrology does not prescribe treatment and does not cancel it. The date of an operation is determined by the doctor, the patient's condition and the hospital's schedule; muhurta applies only within the choice that medicine leaves. Postponing a necessary intervention for the sake of a favourable date is inadmissible — the classical texts say so directly.

The same applies to diagnosis and prognosis: a chart does not replace examination and does not say what a person is ill with. The reasonable use of this branch is to choose a day among those the doctor has offered, and not to schedule a procedure at a plainly unsuitable time when there is a choice.
